STANDARD RATE SCHEDULE: SECURITY LIGHT Rate 17
 
AVAILABILITY:
 
Available upon application to all Members for illumination of outdoor areas at any point on or near the distribution lines where proper secondary voltage is available and subject to the established Terms and Conditions of Service of the Cooperative.
 
MONTHLY RATE:
 
The cost for each 100 watt high pressure sodium lamp installed on Cooperative’s pole with existing facilities is $8.65 per month.  An additional pole and up to 100 feet of secondary conductor may be provided for $3.25 per month.
 
RATE PROVISIONS:
 
The above rate for the lamps only is based on the Cooperative installing the lamp or lamps on existing poles with existing secondary facilities.  For the additional $3.25 per month fee, one pole and up to 100 feet of secondary will be provided.  If additional poles and facilities are required, the Member shall make a contribution equal to the cost of material and labor for the necessary facilities.  All facilities will remain the property of the Cooperative.
 
CONDITIONS OF SERVICE:
 
1.     The Member shall furnish to the Cooperative, without cost to the Cooperative and on forms suitable to it, all rights, permits and easements necessary to permit the installation and maintenance of the Cooperative’s facilities on the Member’s property and as needed by the Cooperative in providing service hereunder.
 
2.     The Member shall be responsible to the Cooperative for damage to its facilities resulting from actions of persons or animals on Member’s premises.
 
3.     Upon receipt of notice from the municipality of failure of light to operate as provided, the Cooperative shall make repairs or replacement within a reasonable time during regular working hours and favorable weather conditions.
 
4.     The facilities furnished by the Cooperative shall remain the property of the Cooperative.
 
5.     The facilities furnished for lighting hereunder shall be installed at a mutually agreeable location on the Member’s premises.
 
6.     The Member shall contract for Security Lighting Service on the standard contract form for a period of not less than one (1) year.
